This is the machform for requesting enrolment in Lab 99 for PHYS courses. Lab 99 spaces will be offered to students in the order in which valid machform submissions are made. You must fill out all required information. You will receive an acknowledgement after submission. Do not submit the form again after receiving the acknowledgement unless you have changed your mind about the course section you want to enroll in. If your submission is valid, you will be contacted with permission to enrol in Lab 99 within five business days. If the course in which you passed the lab component was offered more than two years ago, you cannot enrol in Lab 99 and must redo the lab component.
